Services provided to individuals ages 3-18 in the Youth Empowerment Service (YES) Waiver Program. The YES Waiver provides intensive services within a Wraparound planning process. Performs tasks including linkage, referral, crisis management, and follow-up. A facilitation provides services that are goal-oriented activities that assist clients by locations, coordinating, and monitoring necessary care and services ensuring that they are appropriate and accessible. The facilitator coordinates and facilitates family team meetings for each client and assists in developing and implementing the Plan of Care. Flexible work schedule. Works in a variety of settings.

A minimum of a bachelor’s or advanced degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education or early childhood intervention. Knowledge of community resources; of case management principles, objectives, standards, and methods; and of program policies and procedures. Ability to assess client needs, coordinate client services, and to communicate effectively. Ability to conduct presentations and facilitate meetings. Community-based position with flexible schedule. Valid Texas driver’s license with good driving record.
