Description: 
Performs highly complex (senior-level) case management work. Work involves reviewing and tracking case management activities, and assisting in developing program goals, objectives, and procedures. May assign and/or supervise the work of others. Assists persons with Intellectual and Developments Disabilities (IDD) who are residing in Nursing Facilities (NFs) in developing the Individual Service Plan (ISP), which identifies services and supports needed to obtain life goals.  Completes intakes into services for IDD consumers who are newly admitted to NFs. Monitors consumers’ progress towards outcomes and satisfaction with specialized services and supports. This is completed through monthly face-to-face contact and frequent telephone contact with the individuals assigned, their families, and their Nursing Facilities. 
The work requires intensive coordination of services and available community resources including Medicaid, Social Security, and other community-based programs where available. Provides Community Living Options information to individuals and their families. Completes outreach at NFs to provide opportunities for families and individuals to obtain additional information about Community Living Options. Develops the Community Living Discharge Plan and monitors transitions into the community. 
Also requires interaction and cooperation with various internal departments within Nueces Center for Mental Health and Intellectual Disabilities. Extensive documentation is necessary to ensure the individual receives needed services and makes progress towards their life goals. All documentation must be completed within required timeframes specific to program rules. Staff must be compliant with Center policies and procedures, program protocols and requirements of other regulatory bodies. Works under limited supervision, with considerable latitude for the use of initiative and independent judgement. Will have a schedule that is flexible to the needs of consumers served. 
Must have own transportation. 
Case load is approximately 20-25 individuals.

Minimum Qualifications: 
Bachelors or advanced degree from an accredited college or university with a major in a human service field including, but not limited to, sociology, special education, rehabilitation counseling, and psychology.
Must have experience in case management or social services work.
Minimum of at least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
Knowledge of community resources; of case management principles, objectives, standards, and methods; and of program policies and procedures. 
Ability to assess client needs, to coordinate client services, to communicate effectively, and to train others. 
Must have valid Texas driver’s license and a good driving record.
