Performs moderately complex rehabilitation planning and instructional work. Responsible for delivery of rehabilitative training for assigned caseload; coordination of consumer's needs in order to maintain stability in the community and achieve independence; crisis prevention and intervention. Performs complex professional duties related to the evaluation, individualized program planning. Under general supervision, identifies clients' rehabilitation needs and provides the necessary instruction in teaching self-help, self-care, or other sills according to the needs outlined in the client evaluations, which assist the clients in attaining their optimum levels of independence. Collects, prepares, and reviews case data used to determine client eligibility for financial medical, and/or social services. Services provided to and adults of all ages with a diagnosis of mental illness who exhibit serious emotional, behavioral or mental disorders.
Minimum of a bachelor's or advanced degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education or early childhood intervention.
Valid Texas driver's license with good driving record.
Bilingual in English and Spanish
One year case management experience
Experience working in a mental health setting
One year experience providing rehab services or in teaching and counseling persons with disabilities.
